System Office ID Badges

  • badge reqest form must be completed in order to obtain a new employee, replacement, consultant, or temporary badge.
  • If you lose your ID badge, please see Kelly Tackett in Business Services and pay $16.00 (cash or check); and take your receipt to Kelly Cook in Facilities Management in order to have a new badge completed.
  • If your badge is not working, please email Kelly Cook  and advise her of the problem
  • To request a badge access change, please email your Cabinet member and if the Cabinet head approves the change, Kelly Cook will be advised to make the change
